Nocardiosis is an infection of worldwide distribution and is associated with gram-positive bacilli of the genus Nocardia. The clinical presentation varies widely affecting any body system, mainly the skin and subcutaneous tissues, lungs and central nervous system. The case of an 81-year-old male patient is introduced here. He has a history of arterial hypertension, cognitive impairment, acute myocardial infarction with four stents, abdominal aortic aneurysm and previous cholecystectomy, but with no record of treatments or pathologies that generated immunosuppression. He presents with bacteremia caused by Nocardia farcinica originating from an infectious focus in the skin and soft tissues, which responded favourably to antimicrobial treatment and surgical management of the infectious site.
